Your full date of birth (month/day/year)
Up to three issues — body, mind, and/or spirit — that you are currently concerned with. Please be specific.
If you call HolEssence with your information (815-276-7173), Len or Karen will take your credit card information and charge $40 to your card.
If you email your request for a crystal recommendation, be sure to include your telephone number so that Len or Karen can contact you for your credit card information. For security reasons please do not include your credit card information in the email.
The turnaround time for your personalized crystal recommendation is approximately 3-business days.
